What is an immigration physical?

The exam behind Form I-693

An immigration physical is the medical examination that most green-card applicants must complete as part of their application to register permanent residence or adjust status (Form I-485). Its results are recorded on USCIS Form I-693, Report of Immigration Medical Examination and Vaccination Record.

The purpose of the exam is to show that an applicant is not inadmissible to the United States on health-related grounds. By law, it can only be performed by a physician that USCIS has designated as a Civil Surgeon — and Dr. Sheba Naqvi is a designated Civil Surgeon here at Bald Ridge Family Practice. During the visit, the Civil Surgeon reviews your vaccination history, screens for certain communicable diseases and conditions, and confirms any missing vaccinations. When everything is complete, the Civil Surgeon signs Form I-693 and seals it in an envelope that you submit to USCIS unopened.

This is the practice fee for completing the USCIS Form I-693 examination. It is separate from any filing fee charged by USCIS.

What to bring

  • A valid photo ID
What to expect at your appointment. At your visit, we complete blood work to determine which vaccinations may be missing. Completion of the USCIS immigration forms takes 7–10 business days, after which they are ready for pickup. You will receive your completed Form I-693 in a sealed envelope — please do not open it, as USCIS will not accept the form if the seal is broken.
